Registering A Death

The Death must be registered within 5 working days by the Registrar of Births and Deaths for the area in which it occurred. An appointment system operates at the registrar’s office. You can book an appointment anywhere in Lincolnshire by telephoning (01522) 782244 or online here and follow the ‘online appointment booking service’ link.

Who can Register?
  • Close relative of the deceased
  • Relative in attendance during the last illness
  • A relative living in the district where death occurred
  • A person present at death
  • The person causing the disposal
Documents required
  • Medical Certificate of Cause of Death
  • Medical Card (if available) or
  • Birth Certificate
Information required to Register
  • Date and place of death
  • Full name of deceased (maiden name if applicable)
  • Date and place of birth
  • Occupation and home address
  • If married, full name and occupation of surviving spouse
What the Registrar will give you
  • A certificate for Burial or Cremation (known as the ‘Green’ form) unless the coroner has given you (or issued the Funeral Director) an ‘Order of Burial’ (Form 101) or a Certificate for Cremation (Form 6)
  • Form BD8 - This is for Social Security purposes only. Read the information on the back of the Certificate and send it or bring it to your Social Security Office
